For Independent Contractors, please download, complete, and submit the following:
- W9
- Copy of MC’s certificate from Carrier agency: Carrier shall list BROKER as certificate holder and name BROKER as an Additional Insured and the cargo policy shall include BROKER as a “loss payee”; CARRIER shall provide BROKER with certificates conforming to these requirements and additional insured endorsements.
